After a client is discharged from the hospital, if continued skilled care is needed as they resume their recovery at home, licensed Practical or Registered Nurses referred by 365 In Home Care is your solution. Patients may be transferred from the hospital to their home where our skilled nursing service aide in their recovery.
Registered nurses (RNs) and licensed practical nurses (LPNs) are dedicated to assisting patients in making informed decisions about their health care by offering treatment, education, and critical thinking about their conditions. RNs and LPNs might be referred to help with personal care or elder home health care, manage prescriptions, or offer skilled care as directed by a doctor. The patient’s discharge planner through the patient’s physician will create a medical treatment plan for the family, client, and any other care providers to follow. Aside from the senior home health care services listed below, RNs and LPNs can conduct any other regularly offered service.
- Documents a Medical Treatment Plan (RN only)
- An Activities of Daily Living Plan of Care is assessed, evaluated, and documented (RN only)
- Inform client’s doctor on a regular basis.
- Physical and emotional status of the client is assessed (Any licensed care provider)
- The client’s family and household are assessed to see how they will cope and care for their loved one.
- Carry out the medical plan of treatment prescribed by the physician (both RN and LPN),
- Complex medical issues are kept under observation.
- Equipment assistance
- Catheters
- Tube feeding, peg tube colostomy
- Care for Wounds
- Educating on the importance of caring (suctioning, injections, new diagnosis, etc.)

The skilled care providers we refer has significant training and experience in providing the appropriate degree of individual care and assistance, as well as meticulously monitoring everything from medication to everyday activities.
